Some minor book spoilers, George confirmed recently that a big twist he teased last year involves a certain character who's been killed off in the shows storyline, but not dead in the books story.
Almost certainly Stannis

Stannis is almost certainly dead in the show.
In the books though, I don't think Stannis is dead just yet.

In fairness we never saw Stannis die.
Yes, didn't the tall woman (her name escapes me) hit him with something and the screen went black?

I don't think it'd be a twist if it involved Stoneheart, I do think though that storyline will be violent .
I'm not sure what will happen with Baristan, but hopefully it doesn't involve dying while defending Iraq
